The Accreditation Renewal fees are non-refundable.
You must sign the form by hand, so please print, sign and date it with blue or black ink.
Please return this form with your payment and the other mentioned items to the address below.
Renewal Application of Accreditation is valid for twelve (12) months.
Renewal Fee is due no later than two weeks prior to your anniversary month.
Please return with your application fee check in the amount of $450.00 to:
