Instructions for filling out the membership form:



Please note: you can type directly into this form and then print it. The only part you must sign by hand is your signature and the date at the bottom of both sheets. When you print this form these comment lines will not appear

Once you have filled out this form, simply print it. Sign your name at the bottom and date it. Don't forget to circle whether this is a renewal, or a new Membership at the top of the form.

You must also sign and date the statement of faith, as BOTH are required for membership or renewal.

The address to mail this to along with your check (Please do NOT send cash) is found right after the enrollment form, but before the statement of faith.

STATEMENT OF FAITH

  1. We believe the Bible to be the inspired, infallible, inerrant, and authoritative Word of God.
  2. We believe in one God, eternally existing in three persons, Father, Son and the Holy Spirit.
  3. We believe that man was created in the image of God and that man is born with a sinful nature.
  4. We believe Jesus Christ is the eternal Son of God, and that He entered into the world through the Virgin birth, and lived among us without sin.
  5. We believe Jesus Christ shed His Blood on Calvary as a sacrifice without blemish for all sinners, was buried and rose again bodily, and ascended to the right hand of the Father, from whence He will personally return for His glorified Bride.
  6. We believe people are justified by grace through faith in the shed Blood of the Lord Jesus Christ and not by works.
  7. We believe in a literal heaven and a literal hell. All those who have received Jesus Christ as their personal Savior and Lord will inherit eternal life in Heaven.
